Wi -:ERE INNOVATION MEETS TRADITION
GENERAL NOTES: new construction:
1. The sanitary sewage disposal system shown hereon shall be CONSTRUCTED IN ACCORDANCE with the
requirements of 1995 310 CMR 15 (TITLE V) of the State Environmental Code and local Board of Health
regulations. Any MODIFICATION TO I HIS DESIGN must be approved in writing by the engineer and the local
Board of Health.
2. Contractor shall verify and check the BENCHMARK(S) as shown on this plan prior to construction.
3. Notify the local Board of Hpalth when the system is ready for INSPECTION, prior to any backfilling. If
CONFIRMATION OF CONSTRUCTION (as -built) is required, 3 day advanced notification is required.
4. DEEP TEST HOLE INFORMATION indicates soil condition, percolation rate, and water table elevation at the time
and location of actual testilig and should be verified at the time of construction. The contractor shall notify the local
Board of Health if GROUNQWATER or PERCHED WATER is encountered at a higher elevation than indicated on
this plan.
5. TOP OF FOUNDATION, �.ASEMENT and FIRST FLOOR elevations may be raised but NOT LOWERED without
the consent of the engineer.
6. Unless specified in the Ba6is of Sanitary Design, this system is NOT designed for the use of a garbage grinder or
other high water usage devices.
7. Where the building sewer pipe exits the foundation ABOVE the basement floor, an effluent a grinder pump
conforming to 310 CMR 15.229 may be used to discharge A VOLUME OF LESS THAN 25% OF THE DESIGN
FLOW from any future basement bathroom / sink facilities.
8. If any components of the proposed system are speed as HEAVY DUTY, those components shall conform to all
state and local requirements for ASSHTO H-20 loading.
9. The SEPTIC TANK shall ba 1500 gallons minimum, unless otherwise specified on this design plan, and fitted with
PVC schedule 40 INLET TEE and OUTLET TEE WITH GAS BAFFLE of proper length. Septic tank construction
shall conform to 310 CMR 15.226. The SEPTIC TANK OUTLET COVER is to built up to within 6" of the finished
grade unless otherwise sprcified on this design plan.
10. SEPTIC TANK, DISTRIBUTION BOX, and PUMP CHAMBER (if any) shall be placed on a 6" minimum compacted
GRAVEL BASE to prevent heaving or settling. ALL SEAMS ARE TO BE WATERTIGHT, sealed with asphalt
cement or other cement suitable for that specific component.
11. EXCAVATE ALL UNSUITABLE MATERIAL within five feet horizontally of the leaching area from the pea stone
cover down to elevation El. = 87.3 ( 44 Inches below the original existing grade.) Excavation may be required to
extend deeper if uniform suitable material is not encountered. See notes 11 a & 11 b.
11a &b SOIL PREPARATION PROCEDURE FOR THE LEACHING FACILITY AREA shall. conform to 310 CMR 15.246 &
15.247. Any EXCAVATIO0 OF UNSUITABLE MATERIAL designated on the plan shall conform to Construction in
Fill requirements as outlined in 310 CMR 15.255 (1-6).
12. ANY LEACHING AREA (OR PUMP CHAMBER) VENTS shall be constructed of 4" solvent weld SCH 40 PVC, The
twin 90 degree elbows comprising the "U' shall not be glued. Any D-box vent pipe shall utilize an inlet knockout.
Vent to extend 24" min. above proposed grade.

13. Any CLEAN -OUTS shown shall extend w/in 3" of fin. grade and capped with a SCH 40 threaded clean out fitting.
14. No HEAVY EQUIPMENT shall be run over the components or the prepared leaching area during installation.
RUBBER TIRE MACHINERY are not to be driven over the prepared natural soil base or sand/stone bed during
system installation.
15. TANK SEAMS, riser connections (if any), and all plumbing joints to be installed 100% WATERPROOF. ALL
JOINTS MUST BE WATERTIGHT, sealed with rubber joints, cement or other suitable sealer for that specific
component. Any groundwater or surface water entering the system will severely limit the life span of the System.
16. PUMP CHAMBER is to be a J & R Precast 1000 gallon standard concrete tank, or equal, (unless otherwise
specified) with inlet and outlet locations as specified. Concrete riser (if needed) is to be fitted with waterproof Butyl
asphaltic sealer, mortar, or equivalent. Access cover shall be 20" dia. min. cast iron frame and cover to grade.
16b. PUMP TYPE: Myers MW50, 1/2 hp. with 2.0" NPT discharge. 230 volts recommended if available(115v.
optional), 1 phase, 60hz.
16c. ALARM CONTROL PANEL: Myer Instruments (or approved equal) audible and visual alarm panel, interior mount,
equipped with 230v. pump disconnect circuit. Alarm to be wired to a separate circuit from that of the Pump.
16d. FLOATS: Factory installed "on -off float switch to be field adjusted to specified elevations. Separate MFS
Mechanical Float Switch, with suitable bracket/connection to discharge- PVC, for a4al„r r. To be--hard-wired in junction
box. Any necessary wiring hanging fixture is to be plastic or stainless steel.
16e. JUNCTION BOX: 100% waterproof polypropylene junction box, attached to the end of the electrical conduit, is to
be installed directly under the pump chamber access cover to facilitate future pump, alarm, and float adjustments !
repairs. If riser is used, float & pump connections shall be inside riser. All wiring is to be hard -wired into junction
box. Standard "plug-in" connections not permitted.
16f. PRESSURE LINE: 2" SDR-21 PVC minimum or equivalent. Pressure piping is to exit through unused knockout IF
NO RISER IS REQUIRED. Piping is to exit through riser 18" below cast iron frame if riser is used. Pressure pipe
to be laid to insure efleunt drains back toward the proposed weep hole in the pump chamber to prevent freezing.
16g. Electrical wiring between the dwelling and pump chamber is to be encased in conduit with proper fittings at each
end to maintain a waterproof connection. Electrical installation shall conform with the N. E. C. Code and any
applicable local regulations.
16h. In case of pump failure, the pump chamber and septic tare* are to be pumped by a licensed sewage hauler prior to
repairing of sewage pump. This procedure will ensure rload of the leaching area will occur when pump in
restarted. A warning sign is recommended to be plac it on the pump chamber cover stating: 1. disconnect power
prior to servicing, and 2. proper ventilation to be used wthen working on the pump to prevent a build up of sewer
gases.
16i. A backup generator (adequate for supplying the sewage pump in case of electrical failure) is strongly
recommended
17 EXISTING UNDERGROUND UTILITIES shown are approximate only. Actual locations are to be verified by
Contractor prior to excavation through D.P.W. and Dig -Safe procedures.

GENERAL NOTES: new construction:
1. The sanitary sewage disposal system shown hereon shall be CONSTRUCTED IN ACCORDANCE with the
requirements of 1995 310 CMR 15 (TITLE V) of the State Environmental Code and local Board of Health
regulations. Any MODIFICATION TO THIS DESIGN must be approved in writing by the engineer and the local
Board of Health.
2. Contractor shall verify and check the BENCHMARK(S) as shown on this plan prior to construction.
3. Notify the V)Cal Board of Health when the system is ready for INSPECTION, prior to any backfilling. If
CONFIRM;TION OF CONSTRUCTION (as -built) is required, 3 day advanced notification is required.
4. DEEP TEST HOLE INFORMATION indicates soil condition, percolation rate, and water table elevation at the time
and location of actual testing and should be verified at the time of construction. The contractor shall notify the local
Board of Health if GROUNDWATER or PERCHED WATER is encountered at a higher elevation than indicated on
this plan.
5. TOP OF FOUNDATION, BASEMENT and FIRST FLOOR elevations may be raised but NOT LOWERED without
the consent of the engineer.
6. Unless specified in the Basis of Sanitary Design, this system is NOT designed for the use of a garbage grinder or
other high water usage devices.
7. Where the building sewer pipe exits the foundation ABOVE the basement floor, an effluent a grinder pump
conforming to 310 CMR 15.229 may be used to discharge A VOLUME OF LESS THAN 25% OF THE DESIGN
FLOW front any future basement bathroom / sink facilities.
S. If any components of the proposed system are specified as HEAVY DUTY, those components shall conform to all
state and local requirements for ASSHTO H-20 loading.
9. The SEPTIC TANK shall be 1500 gallons minimum, unless otherwise specified on this design plan, and fitted with
PVC schedule 40 INLET TEE and OUTLET TEE WITH GAS BAFFLE of proper length. Septic tank construction
shall confo rn to 310 CMR 15.226. The SEPTIC TANK OUTLET COVER is to built up to within 6" of the finished
grade unless otherwise speed on this design plan.
10. SEPTIC TANK, DISTRIBUTION BOX, and PUMP CHAMBER (if any) shall be placed on a 6" minimum compacted
GRAVEL EASE to prevent heaving or settling. ALL SEAMS ARE TO BE WATERTIGHT, sealed with asphalt
cement or ether cement suitable for that specific component.
11. EXCAVATi= ALL UNSUITABLE MATERIAL within five feet horizontally of the leaching area from the pea stone
cover down to elevation El. = 87.3 ( 44 Inches below the original existing grade.) Excavation may be required to
extend deeper if uniform suitable material is not encountered. See notes 11 a & 11 b.
11a &b SOIL PREPARATION PROCEDURE FOR THE LEACHING FACILITY AREA shall. conform to 310 CMR 15.246 &
15.247. Any EXCAVATION OF UNSUITABLE MATERIAL designated on the plan shall conform to Construction in
Fill requirements as outlined in 310 CMR 15.255 (1-6).
12. ANY LEACHING AREA (OR PUMP CHAMBER) VENTS shall be constructed of 4" solvent weld SCH 40 PVC, The
twin 90 dedree elbows comprising the "U" shall not be glued. Any D-box vent pipe shall utilize an inlet knockout.
Vent to exiand 24" min. above proposed grade.

13. Any CLEAN -OUTS shown shall extend w/in 3" of fin. grade and capped with a SCH 40 threaded clean out fitting.
14. No HEAVY EQUIPMENT shall be run over the components or the prepared leaching area during installation.
RUBBER TIRE MACHINERY are not to be driven over the prepared natural soil base or sand/stone bed during
system installation.
15. TANK SEAMS, riser connections (if any), and all plumbing joints to be installed 100% WATERPROOF. ALL
JOINTS MUST BE WATERTIGHT, sealed with rubber joints, cement or other suitable sealer for that specific
component. Any groundwater or surface water entering the system will severely limit the life span of the System.
16. PUMP CHAMBER is to be a J & R Precast 1500 gallon standard concrete tank, or equal, (unless otherwise
specified) with inlet and outlet locations as specified. Concrete riser (if needed) is to be fitted with waterproof Butyl
asphaltic sealer, mortar, or equivalent. Access cover shall be 20" dia. min. cast iron frame and cover to grade.
16b. PUMP TYPE: Myers MW50, 1/2 hp. with 2.0" NPT discharge. 230 volts recommended if available(115v.
optional), 1 phase, 60hz.
16c. ALARM CONTROL PANEL: Myer instruments (or approved equal) audible and visual alarm panel, interior mount,
equipped with 230v. pump disconnect circuit. Alarm to be wired to a separate circuit from that of the Pump.
16d. FLOATS: Factory installed "on -off' float switch to be field adjusted to specified elevations. Separate MFS
Mechanical Float Switch, with suitable bracket/connection to dischargeQVC, for alarm. To b2-hard-wired in junction
box. Any necessary wiring hanging fixture is to be plastic or stainless steel.
16e. JUNCTION BOX: 100% waterproof polypropylene junction box, attached to the end of the electrical conduit, is to
be installed directly under the pump chamber access cover to facilitate future pump, alarm, and float adjustments /
repairs. If riser is used, float & pump connections shall be inside riser. All wiring is to be hard -wired into junction
box. Standard "plug-in" connections not permitted.
16f. PRESSURE LINE: 2" SDR-21 PVC minimum or equivalent. Pressure piping is to exit through unused knockout IF
NO RISER IS REQUIRED. Piping is to exit through riser 18" below cast iron frame if riser is used. Pressure pipe
to be laid to insure efleunt drains back toward the proposed weep hole in the pump chamber to prevent freezing.
16g. Electrical wiring between the dwelling and pump chamber is to be encased in conduit with proper fittings at each
end to maintain a waterproof connection. Electrical installation shall conform with the N. E. C. Code and any
applicable local regulations.
16h. In case of pump failure, the pump chamber and septic tanr, are to be pumped b, a licensed sewage hauler prior to
repairing of sewage pump. This procedure will ensure .'.5rioad of the leaching area will occur when pump in
restarted. A warning sign is recommended to be place,"'he pump chamber cover stating: 1. disconnect power
prior to servicing, and 2. proper ventilation to be used v0hri working on the pum;r to prevent a build up of sewer
gases.
16i. A backup generator (adequate for supplying the sewage pump in case of electrical failure) is strongly
recommended
17 EXISTING UNDERGROUND UTILITIES shown are approximate only. Actual locations are to be verified by
Contractor prior to excavation through D,P.W. and Dig -Safe procedures.
