Annual Maintenance 1. Inspect and clean catch basins preceding the STS tanks. 2. Visually inspect influent pipe and clean out debris, if necessary. 3. Open manhole cover. 4. Remove debris filter sack and attach replacement. 5. Visually inspect skimmers to ensure that the flexible hoses are undamaged and tightly connected to the skimmer and bulkhead. Replace damaged hoses. 6. Close manhole cover. 7. Collect debris from wetland area and trim dead growth from wetland plants. Replace dead plants as necessary. ' 8. Rake and clean up area around systems. 9. If there is flow from the effluent pipe, open valve cover, measure flow rate and adjust. Reset exit valve if necessary. Close and lock valve cover. I1. Haybales will be maintained throughout the construction stages and until all disturbed soil areas are stabilized with new vegetation. 5. Any substantial silt accumulation against the haybale line will be removed by hand labor to prevent the possibility of over washing the checkdam. 7. Stockpiled earth materials are to be placed no closer than fifty feet from the Resource Area. 8. No refueling of construction equipment will be performed within 100 feet of the Resource Area. 9. All disturbed areas will be brought to finish grade with 6 inches of loam and seeded. 11. Prior to the start of any construction activities, this Order of Conditions shall be recorded at the Bristol County (S.D.) Registry of Deeds in accordance with General Condition 48. The recording information found on the bottom of page 54A shall be completed, stamped with recording information by the Registry, and returned to the Conservation Commission. 12. A stockpile of a minimum of fifty (50) haybales and sufficient stakes for installing these haybales shall be maintained on site for use if necessary to control erosion problems in the event of an emergency situation. 13. It is the responsibility of the Owner and/or successors and assigns to ensure that all conditions for performing the permitted work are complied with. Copies of the Order of Conditions and approved design plans referenced by the Order of Conditions must be provided to all contractors working within the Buffer to the Resource Area before they are permitted to start work. 14. _ Prior to the start of any construction activity, the contractor shall contact the Dartmouth Conservation Commission Agent to make notification of his intent to begin work. A pre - construction meeting may be required to discuss any specific requirements for work within the Buffer Zone.
